Card on File: A valid credit card must be provided and will be securely stored on file. This card will be charged for appointments that are rescheduled or canceled in under 48 hours and No-Show appointments. Please note that any changes for Monday’s appointment must be made the Friday prior by noon. By scheduling an appointment with Destin Medical Spa, you acknowledge that you have read, understood, and agree to all terms outlined below.
Rescheduling: Should any event occur where you must cancel or reschedule your appointment, please notify us 48 hours in advance for appointments Tuesday through Friday and 72 hours in advance for Monday appointments. Changes for Monday appointments must be made the Friday prior before noon. All appointments canceled with less than proper notice will incur a $50 reschedule or cancellation fee to be charged to your authorized card on file. By scheduling your appointment, you are agreeing to our cancelation policy.
Reschedule Limit: You are permitted to reschedule your appointment up to three times. Any patient who misses 3 scheduled appointments without providing proper notice will be subject to dismissal from the practice.
No-Show Policy: Failure to attend your scheduled appointment without prior notification (No-Show) will result in a non-refundable $50 no show fee.
Medical Emergencies: If you are unable to attend your appointment due to a medical emergency, proper medical documentation must be provided to be considered to have any late fees waived.
